How do we make a portal from one end to the other?

Question

I was wondering... when I go to multiplayer servers sometimes i see portals leading to one end to the other and thats cool. So I was asking you guys if you know how to make a portal that leads to where you want it to lead.

Example: So lets say I want to make a portal that looks like a nether portal on a dessert and I go to a forest and climb a tall tree and make another nether portal on top of the tree. Now I go in the portal on the dessert and when I go in the portal it leads to the portal I put on the tree (not the nether)

 

 

So can a nice person tell me how i can do this? Please.... Do we need like a command block or something?

You are going to need to install a mod or plugin.  I think you can do this with a few multiverse plugins.  There are many tutorials online.
