For a Nether portal you need to make a 4x5 rectangle made of obsidian standing up then light it with flint and steel. For an End portal you need to find a stonghold and it should be a floating square made of some odd looking things above a pool of lava. Fill the empty spots of the portal pieces with ender eyes and it should be ready to go.
