    How does one go at reporting a server on the forms that break rules and terms?

    This is not a thing. You might be able to get an offensive server removed from server listings, but otherwise, every Minecraft server is an independently owned and operated service. Server owners purchase their own hosting and domains, and as such they can generally not be taken down in the way you are probably thinking. Even if Mojang, the developers of Minecraft, gave up on the game, servers would continue operating because of the way they work.

    If this is troubling to you, I hope you find peace in the fact that most servers owned by incompetent or abusive administrations do not find success. Such servers typically stop gaining players, and close down after a few weeks of failure. This is entirely from my own experiences.
